As we all prepare to take on this invisible threat, our team at AMI has been working diligently to make sure you have access to surgical care. Through Tele-Surgery, we are prepared to continue our patient visits and consultations using a secure HIPPA compliant web-based media platform. All our surgeons and physician assistants will be able to connect directly with you, face to face, to discuss and review any concerns you may have. If you need to be seen, this can be discussed through our Tele-Surgery platform and a one-one visit can be arranged in the office within 24-48hr period.

BEFORE YOUR INITIAL VISIT/CONSULTATION

  • Educate yourself about weight loss surgery!
  • Ask you primary care physician for a referral to our office
  • It is important that YOU call your insurance company and verify that you have coverage for weight loss surgery (we will do this as well)

 

DURING YOUR INITIAL VISIT/CONSULTATION

  • This visit will last approximately 30-40 minutes
  • Bring your insurance card
  • Bring a complete copy of all your medical records (if available) [this can be faxed or mailed to us]
  • You will meet with Dr. Bonheur to discuss your options and goals for surgical weight loss
  • You will be given the bariatric folder containing useful information about our program.
  • Make your second appointment to have a physical exam with Dr. Bonheur
  • Begin to make your appointments to see the required medical consultants

 

DURING YOUR SECOND VISIT

  • This visit will last approximately 40-60 minutes
  • Bring the completed health information packet (this MUST be filled out prior to this visit)
  • You will meet with Dr. Bonheur and undergo a physical exam and he will review any reports already forwarded from your medical consultants.
  • Pay Commitment fee
  • Plan to attend a support group meeting and our monthly weight loss seminar
  • Address any questions  with Dr. Bonheur, the Bariatric Coordinator, or insurance specialist
  • Confirm insurance requirements and approval for surgery
  • Obtain a tentative date and location for surgery

 

DURING YOUR THIRD VISIT [2 WEEKS BEFORE SURGERY]

  • This visit will last approximately 30 minutes
  • Discuss any final concerns regarding upcoming surgery
  • Dr. Bonheur will review all reports forwarded from your medical consultants and discuss the risks and benefits associated with surgery
  • Start pre-operative diet
